Walk into almost any fertility clinic today even one marketed as the Best IVF Centre in its region and, alongside the standard IVF protocol, you’ll likely be offered a menu of optional extras: endometrial scratching, embryo glue, time-lapse imaging, immune therapies, PGT-A genetic screening, acupuncture, and more. These “add-ons” can increase the cost of a single IVF cycle by hundreds or even thousands of dollars but how many of them actually improve the odds of having a baby? This is a look at what the evidence really says, and why the gap between marketing and science has become one of the most quietly controversial issues in fertility medicine.
Why Add-Ons Exist
IVF is expensive, emotionally taxing, and doesn’t always work on the first try. That combination creates a powerful incentive on both sides: patients are often willing to try almost anything that might improve their chances, and clinics operate in a competitive, largely private-pay market where offering the latest technology can be a selling point. Many add-ons are approved for use meaning they’re safe without being proven to actually increase live birth rates, which are the outcome that matters most.
This distinction between “approved” and “proven effective” is where much of the controversy lives.
What the Evidence Actually Shows
Independent reviews, most notably the UK’s Human Fertilisation and Embryology Authority (HFEA), which rates common add-ons using a traffic-light system based on the quality of supporting evidence have found that the large majority of popular add-ons lack robust evidence of benefit for the average patient.

- PGT-A (preimplantation genetic testing for aneuploidy), which screens embryos for chromosomal abnormalities before transfer, is genuinely useful in specific situations for instance, for some patients with recurrent miscarriage or advanced maternal age but its benefit for the general IVF population is still debated, and it doesn’t guarantee a healthy pregnancy.
- “Embryo glue” (a culture medium used during transfer) has shown mixed results across studies, with some suggesting a modest benefit and others finding none.

In that state, the promise of even a small edge can feel worth almost any price. Clinics aren’t necessarily acting in bad faith; many genuinely believe in the interventions they offer, and the science in this field moves quickly. But the asymmetry of information between clinic and patient means it can be difficult for someone outside the field to know which recommendations are backed by strong data and which are, at best, promising but unproven.
What This Means for Patients

- Check independent ratings where available. Resources like the HFEA’s add-on ratings (for UK patients) offer a rare independent look at the evidence base, though similar centralized resources are less standardized in other countries.
- Look beyond marketing labels. A clinic calling itself the Best IVF Centre in its city or country isn’t a substitute for checking its actual published success rates and evidence-based practices.
- Consider cost relative to benefit. An add-on that costs relatively little and carries no meaningful risk may be a reasonable choice even with modest evidence. One that adds thousands of dollars with uncertain benefit deserves more scrutiny.
- Remember that “newer” isn’t automatically “better.” Novel technology sometimes represents real progress, and sometimes represents a good story that hasn’t yet been tested rigorously.
A Field Still Catching Up to Itself
The IVF add-on debate reflects a broader tension in reproductive medicine: the science of assisted reproduction has advanced rapidly, but the infrastructure for rigorously testing every new refinement hasn’t always kept pace with the enthusiasm for offering it.
